This Privacy Policy describes how Tria Health LLC, doing business as TriaPath (“TriaPath,” “we,” “us”), handles information collected through this website (triapath.com). It does not cover the health records we create and maintain once you become a client. That information is governed by our HIPAA Notice of Privacy Practices, provided separately at intake.
Information we collect
When you submit our intake form, we collect the information you choose to provide, such as your name, phone number, email address, preferred service, Medicaid plan, preferred location, and any optional message. We ask that you not include medical, diagnostic, or other clinical details in this form.
If you call or email us, we receive the information you choose to share in that conversation.
Our intake form
Our intake form is provided by Jotform, a third-party form service, and is shown on this site in an embedded frame. TriaPath uses Jotform’s HIPAA-enabled service for this form. What you submit is sent through Jotform to TriaPath’s intake team.
When the form loads, Jotform may collect technical information and set its own cookies, as described in Jotform’s privacy policy. Your use of the form is also subject to Jotform’s terms of use.
How we use it
We use this information solely to respond to your request, verify insurance benefits, and schedule an appointment. We do not sell this information, and we do not use it for advertising or share it with third-party ad networks.
Cookies and analytics
This website does not use analytics, advertising, or tracking cookies, and it does not load third-party analytics or advertising scripts.
It sets one functional cookie, NEXT_LOCALE, to remember the language you choose (English or Spanish). It contains no personal information, and you can clear it at any time in your browser settings.
Like most websites, our hosting provider may keep basic technical logs (such as IP address, browser type, and the pages requested) to keep the site secure and working.

TriaPath complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex. We provide free language assistance services to people whose primary language is not English. Ask our intake team.
