This Privacy Policy describes how Tria Health LLC, doing business as TriaPath ("TriaPath," "we," "us"), handles information collected through this website (https://triapath.com). It does not cover the health records we create and maintain once you become a client — that information is governed by our HIPAA Notice of Privacy Practices, provided separately at intake.

When you submit our appointment request form, we collect the information you provide: your name, phone number, email address, preferred service, insurance/payer, preferred location, and any optional message. We ask that you not include medical, diagnostic, or other clinical details in this form.

How we use it

We use this information solely to respond to your request, verify insurance benefits, and schedule an appointment. We do not sell this information, and we do not use it for advertising or share it with third-party ad networks.

Analytics

We use privacy-focused, cookie-free website analytics to understand overall site usage (such as which pages are visited). This analytics tool does not track you individually across other websites and does not receive the contents of your form submissions.
